New Patient Registration Form
Complete your personal details, contact information, and dental history before your first visit. Submitting this in advance means more time for care — less time on paperwork.
Medical History Form
Your health history is essential to safe, personalised care. Please disclose all current medications, allergies, and medical conditions so your dentist can tailor treatment accordingly.
Dental Consent Form
Informed consent for standard dental examinations, X-rays, and preventive treatment. Specific consent forms for surgical or complex procedures are provided at your appointment.
Provided and signed at your appointment
Insurance & Billing Authorization
Authorise Alces Dental to submit claims directly to your insurance provider and assign benefits to the practice. Speeds up processing and reduces your out-of-pocket wait.
PDF forms require Adobe Acrobat Reader or any standard PDF viewer. Once complete, print and bring your form to your appointment, or email it to us at hello@alcesdental.com. We will keep a copy on file.
Before You
Submit Your Forms
- Complete forms at least 24 hours before your appointment when possible
- Have your insurance card and policy number ready when filling out billing forms
- List all medications including vitamins, supplements, and over-the-counter drugs
- If you have existing dental X-rays, let us know — we can request them from your previous dentist
- Call us if you have any questions while completing the forms — we are always happy to help
