What to Bring
to Your Appointment
Arriving prepared helps us spend more time on what matters most — your care. Here is what to bring to your first appointment at Alces Dental.
- Government-issued photo ID (driver's licence, passport)
- Provincial health card
- Insurance card and policy number (if applicable)
- CDCP benefit card issued by Sun Life (CDCP patients)
- List of current medications and dosages
- Any relevant dental records or X-rays from a previous dentist
- Completed new patient intake form (or arrive 10 min early)
